Job summary

An exciting opportunity for experienced Registered Band 6 Practitioner to work within a dynamic multi-disciplinary team, who provide Community Rehabilitation Services to the Mental Health and Learning Disability Division for the areas of North Wales as part of Specialist Commissioning Care Services.

Main duties of the job

The team is based in central area of Denbigh The appointed candidate will hold a caseload and act as Care co-ordinator under the Mental Health (Wales) Measures Framework, delivering rehabilitation focused services for service users within NHS rehabilitation inpatient units, Out of Area placements such as independent hospitals, low secure units, specialist placements and also within the community.

The aim of the team is to provide a focus on the repatriation of services users back to their home area, and also to provide intensive community rehabilitation to enable Service Users with Complex Emotional Needs and or Complex Rehabilitation needs to remain within their local area

This post is open to experienced registered practitioner such as Mental Health Nurses and Social Workers that meet the requirements of the post

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

  • Professional Qualification in relation to Adult Mental Health/Educated to degree level
  • Evidence of continued professional development
  • Demonstrate competency of post qualifying experience in mental health and / or able to demonstrate substantial experience in the assessment rehabilitation and management of service users requiring long term management within the community
  • Team working skills/Good knowledge of mental health assessment and assessment tools oEvidence of ability to work within identified risk areas in a positive manner utilising team support. oCommitted to raising standards in mental health services oAbility to prioritise and work proactively oEffective verbal and non verbal communication skills. oAbility to work in a pressurised environment oAbility to work with service users / carers as equal contributors to service planning and delivery. oAbility to see service users and carers as equal partners in the management of their own care and support. oAbility to supervise and manager Junior members of staff
  • Awareness of current developments within MH &LD programme and also within BCULHB. oUnderstanding of Clinical Governance oKnowledge of and commitment to the recovery and social inclusion of clients. oKnowledge of Child Protection and SOVA issues oKnowledge and awareness of all Wales Mental Health Measures
