Job summary

A permanent opportunity has arisen to join the Surgical Team as a Band 3 Progress chaser. Applications are welcomed from both clinical and non-clinical staff.

Role Summary:

The post holder will provide a service for the Medical and Surgical Directorates. They will be an integral component of the Directorate Teams, working closely with the Matrons and Ward Staff to facilitate a safe and effective timely discharge/transfer and review of 'outlying' patients from hospital. The post holder will also need to work alongside the Clinical Site Managers to ensure that bed use is maximised and support with prompt and safe discharge. This will include;o Capture all discharge data for day.o Chase all outstanding actions.o Liaise with Progress Chasers for any actions that need completing today.o Chase community bed transfers.o Update discharge spreadsheet.o Identify Golden patients for next day early discharge.o Offer support to wards to facilitate timely and safe discharges.o Identify and highlight out of speciality patients.o Escalate any concerns to B6 .

Main duties of the job

To support effective patient management for our wards in Surgery and Medicine, we are looking to recruit a Progress Chaser to join our team. This is an exciting opportunity for a dynamic individual who will be a key resource in supporting our ward teams.

The post involves working in a multi-disciplinary environment and will support systems and processes to positively impact patient management.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
