Job summary

Are you a Mental Health nurse looking for a new and exciting challenge?

We are looking to recruit an enthusiastic and motivated band 6 mental health nurse to work with us in the South Gwynedd older persons mental health team. The successful applicant will be enthusiastic, motivated, has a flexible approach to work, and passionate about improving services for people with dementia. As care co-ordinator, the post holder will be working closely with other agencies such as social services to support people in their community, whether that be in their own home, in hospital, or in a care home. This is a full time, permanent contract, based in Hafod Hedd, Ysbyty Bryn Beryl.

Main duties of the job

The post holder is required to work within an integrated MDT focusing on older adults suffering organic mental health problems (dementia). The post holder will hold a caseload, being responsible for assessing patients' needs, planning and implementing the plan of care.

The post holder will provide supervision, guidance and support to junior members of staff.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
