Job summary
The Home First Team is a service that provides the Discharge to Assess pathway for the East IHC. The Home First Therapy Team offer an assessment to patients who are discharged to their own home and require this within 48 hours of discharge. The Home First Team will provide ongoing therapeutic reablement to patients within their own home to increase independence and right size care needs within the community.
Main duties of the job
This is an exciting opportunity for a band 6 Physiotherapist to join the East IHC Home First Service.
The role will compromise of Physiotherapy assessment and treatment within the specialty for clients who may have complex and acute or chronic presentation and to determine therapeutic interventions within their own home based on evidence based/best practice.
You will work in partnership with the other members of the internal and extended multidisciplinary team to enable patients to receive a comprehensive, holistic service.
You will provide leadership and supervision for junior staff, generic OT/ PT Technical instructors, and students on clinical placement.
You will participate in the planning, development, and evaluation of the service, supporting the clinical specialist therapist with operational day to day management of the service.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
