Specialist Occupational Therapist - Primary care Conwy Area Cluster
BAND 6: Part Time (7.5 hours a week)
An exciting opportunity has arisen to join the Occupational Therapy team in Conwy cluster area. Come and be part of a new and exciting team where you will promoting the role of the occupational therapist. Working in primary care offers you to practice a multitude of skills; dealing with a wide range of physical and mental health conditions.
The focus of primary care is getting people to live healthier lives for longer. We are looking for someone enthusiastic, keen to develop and evaluate services and who is a skilled facilitator in self-management and familiar with social prescribing and partnership working.
The base for the vacancy is to be confirmed but may be negotiable for the successful candidate.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job
The post holder will be responsible for providing specialist Occupational Therapy assessments and a range of individual and/or group interventions drawn from relevant frames of reference, demonstrate safe, clinical decision-making skills to provide expert care for patients within the general practice. They will prioritise planning and delivering assessments and interventions for service users whose physical and mental health problems are having a marked impact on their occupational functioning and who have complex occupational needs.

The post holder will be expected to work towards the development of expertise in areas of primary care . This will include the link between physical and mental health and its correlation with health and well-being and ensures this is considered as part of psychosocial assessment.
The post-holder will research and use OT assessments and interventions pertinent to this client group, and be able to demonstrate measurable outcomes, to develop the OT services into this fieldClinical activities can take place in a wide range of settings including GP surgeries, clinic and community settings, and patients own homes.
The post holder will provide supervision to Occupational Therapy students and in addition may provide supervision or guidance to other professionals from other disciplines. The post holder will support the learning and development of others including colleagues and peers.
The post holder will contribute to and lead some areas of service development duties within primary care which can include attending meetings and leading in, audit, evidence based practice research and service evaluation.
The post holder will work in partnership with patients, other health and social care colleagues and community agencies as appropriate, to achieve the optimum health outcome for patients.
