Job summary
To provide person centered nursing assessment and planned therapeutic interventions and evaluations under the supervision of senior clinicians.
To describe and maintain enabling environments which are sensitive to the highly individualised communication needs of the individual patient.
To contribute to the implementation of clinical governance within the hospital.
To actively contribute to education training and professional development within the BCUHB and Bangor University.
To participate in the management of TSS staff
Main duties of the job
To work directly with patients, 1:1 and in groups, to support involvement in recreational, educational and social activities in accordance with individual treatment programmes. To develop clients narratives within their care and treatment plan. To assess, plan, implement and evaluate nursing care using person centred approaches making and using specialised communication tools in enabling environments. Monitor and take steps to ensure and maintain a safe environment that meets the needs of patients and staff. As part of the nursing team at TSS and under supervision of senior clinicians, to offer patients a variety of different strategies and coping mechanisms to promote mental health and physical wellbeing. Meet regularly with allocated clients during their inpatient stay. Working collaboratively with patients, their families and other members of the MDT, to formulate and further develop risk assessments and management plans which embrace a human rights perspective. To ensure accurate contemporaneous care records are written and maintained (including electronic records) and to prepare reports and discharge summaries. To communicate information of a sensitive, contentious and confidential nature to clients, members of their family and other members of the MDT overcoming barriers to communication by facilitating communication using augmented or specialised communication systems including multi media.
