Job summary
The post holder will be responsible for the management and the futuredevelopment of a high quality, accessible nurse led service. Functioning closely with colleagues within specialised areas thereby co-ordinating care with the multi-disciplinary team within the Betsi Cadwaladr University Health Board and be the designated named lead nurse within the team.Demonstrates advanced and specialist practice, providing leadership, direction, support and advice during a multi-professional review of patients on the ward who have triggered the early warning score, patients referred who have recently been discharged from Critical Care. To be responsible for undertaking comprehensive, holistic assessments of critically ill patients, planning, andevaluating care delivery for patients on the wards who are at risk of deteriorating from Level 1 into the need for Level 2 or 3 critical care. To provide highly specialised interventions and support with appropriate physical assessment, observation and treatment.
Therefore enabling through direct and indirect activity the monitoring and improving of standards of care through supervision of practice, clinical audit, development of practice through research, teaching and the support of professional colleagues and the provision of skilled professional leadership.
Main duties of the job
The post holder will be able to demonstrate advanced and specialist nursing practice in holistic assessment, planning, implementation and evaluation of care for emergency and elective patients and their families, according to changing care needs. This will necessitate high levels of autonomy and advocacy. Eg to work independently in a variety of multi-speciality clinical settings hospital wide.Direct, influence, advise and facilitate the discharge of patients from the critical care unit and theatre/recovery to general wards. Initiate and direct the stabilization and transfer of critically ill patients to the appropriate environment including inter-hospital transfers by road and air ofLevel 2 and Level 3T patient covering all aspects of communication in the multi-professional environment.The post holder will perform a comprehensive patient problem focused health history, physical examination, analyses and interprets history presenting symptoms, physical findings and diagnostic information to develop differential diagnosis within a specialist area.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
