The origins of clusters (formerly Localities) can be found in the Welsh Government (WG) document, Setting the Direction.

WG published subsequent key documents outlining the vision of clusters as influential and essential in the delivery of primary and community care, contributing to the integrated medium term plan (IMTP) of the Health Board:

  • Our Plan for a Primary Care Service for Wales up to March 2018
  • A Healthier Wales: Our plan for Health and Social Care

In 2018, BCUHB published its 3-year plan, which outlined the Care Closer to Home strategy.

In addition, the Accelerated Cluster Development (ACD) programme is underway.

Against this background, Conwy West Cluster continues to evolve and is on the cusp of exciting change and development.
