Job summary
The post holder is responsible for the delivery of high quality nursing care to the general practitioner patient population. The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care.
They will be required to liaise with nursing and medical colleagues and other members of the Multidisciplinary Health Care team to ensure the delivery of effective patient care. Supported by various colleagues within practices and hospitals, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, offer advice and aid in the monitoring of long-term conditions and health promotion activities. The post holder will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of care whilst adhering to current policy and procedures.

The Primary Care Treatment Centre currently based in Llandudno are looking to recruit an enthusiastic and experienced RN. This is full time permanent clinic post initially It is essential for you to have experience with all aspects of wound care including Lymphoedema and leg ulcer, trauma and Post-Operative wounds. PCTC provide a range of treatments including catheterisation, stoma care and Ear Irrigation, prior experience is an advantage.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
