Job summary

We have an exciting opportunity for a forward thinking clinical pharmacist to develop their career working in a GP practice.

We would welcome a motivated clinical pharmacist passionate about patient care, who has excellent communication skills and is able to work in a multi-disciplinary team to optimise medications with a patient-centred approach. You will be based at Tywyn Health Centre and will be an integral member of the team helping us provide high quality innovative patient care

Experience of working within GP surgery is desirable.

Main duties of the job

This role is for a prescriber who will have the opportunity to develop pharmacist led chronic disease clinics. You will be expected to lead and coordinate the Medicines Management Team on a day to day basis, take an active role in supporting the multi-disciplinary team with the surgery's weekly care home reviews, as well as supporting chronic disease management and year of care.

The post is open to pharmacists working in all sectors of the profession with suitable experience and qualifications.

Appointment to post will be at Band 8a. Applications for job shares will be considered. Reimbursement of relocation expenses may be available as per Health board policy.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
