Job summary

The post holder will work closely with the dementia development team to utilise knowledge and skills so as to enable other staff across the hospital site to deliver, safe effective and compassionate care to people with dementia, and support to their carers and families.

Main duties of the job

An exciting opportunity has arisen for a Band 3 Activity/Dementia Support Worker to work across the site at Alltwen Hospital.

The new Activity/Dementia Support worker post holder will work as part of the multi-disciplinary team in order to plan, organise and facilitate a comprehensive range of individualised, group and one to one activities for the inpatients in the Eryri Hospital.

The post holder will be expected to support patients on both wards. They will promote and encourage appropriate, individualised levels of patient participation and facilitate patient activities that promote social interaction, enhance self-esteem and help to maintain or enhance functional ability.

Applicants for this vacancy need to have evidence ofpatient participation and the ability to care with warmth and understanding. The role will be supporting nursing staff to ensure that the patients receive the best possible outcome and experience during their hospital stay.

The successful post holder will be required to support the Ward Senior Sister / Charge Nurse & Ward nursing team through ensuring high standards of care as outlined in the Welsh Government Fundamentals of Care.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
