Job summary
The post holder will be based within the Liaison Team and will be responsible for organising their professional day and for ensuring that they can be contacted at all times during working hours.
The post holder will support the Liaison Team with input to Ysbyty Glan Clwyd, the local DGH. Starting from the single point of access, liaison nursing staff will screen referrals and allocate appropriately to adult, older adult or alcohol liaison Band 6 nurses. Sometimes assessments are carried out jointly including with the team Occupational Therapist or Specialty doctor. The post holder will help with assessments where required and supervise assessments helping with diagnosis/formulation of patients' problems and ensuring a clear management plan is available and well documented in patients' notes and communicated with the relevant teams.

Main duties of the job
The post holder will provide specialist psychiatric assessment, treatment and management for adults aged 18 upwards presenting to the district general hospital. The post holder will provide consultation and supervision for the liaison team.
The post holder will be based with the liaison team and work with the team leader to ensure clinical leadership of the multi-disciplinary team. The post holder will assist with clinical supervision of the team. It is anticipated that the post holder will assist in training of team members in clinical matters.
The service operates an Acute Care Model with 2 dedicated inpatient consultants managing patients admitted to the inpatient unit. The inpatient consultants also have responsibility for the Home Treatment Team patients. The post holder will work collaboratively with the Acute Care Services and the other Liaison consultant.
