An exciting opportunity has arisen for a Band 6 Community Psychiatric Nurse to work within the Conwy Community Mental Health Team for Older Persons.

The post is to work with the older person client group who are presenting with symptoms suggestive of an organic illness (dementia, cognitive impairment) and functional illness such as anxiety and depression. The CPN will be responsible for assessing, implementing and evaluating care and the risk management of persons over the age of 70 who live in the Conwy area. They will work closely with the Consultant Psychiatrist, Social Services, and in-patient wards as well as third sector and voluntary agencies to ensure the patients are receiving the right care in the right environment.
