Job summary

An exciting opportunity has arisen for an experienced and proactive Occupational Therapist to join our in-patient team within Llandudno General Hospital.

The role will predominantly involve working with patients on Discharge to Recover and Assess ( D2RA) Pathway 3 in a Community Hospital setting. You will be working as part of a multidisciplinary team to provide assessment and rehabilitation for people with complex physical and mental health needs to support discharge planning.

Main duties of the job

You will have experience of working with people who have complex needs and working to a home first approach to minimise deconditioning and maximise independence.

You will have excellent communication, organisational and time management skills and be able to work flexibly to meet the demands of this role. You will be supporting junior team members with both informal and formal supervision and have the opportunity to develop your skills in clinical leadership whilst being supported by the Senior Occupational Therapy Team. There will be opportunities to be involved with service developments as we further develop D2RA pathways.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
