Job summary
The postholder will be a member of the Community Learning Disability team, working closely with the multi- disciplinary team. Responsibilities include the maintaining of a clinical caseload and conducting assessments of highly complex problems which require the analysis, interpretation and comparison of a range of solutions for adults with learning disabilities and their families who are experiencing complex health difficulties, associated with their learning disability.
This role will include the maintaining of clinical standards within the governance framework.
Main duties of the job
To work within their code of practice to take responsibility for their caseload, ensuring comprehensive assessments of their needs. This includesthe formulation, planning, delivery and evaluation of care and will involve the regular review and updating of risk assessments, assessmentdocumentation and care plan reviews.To work collaboratively across a wide range of multi-disciplinary services and agencies to provide effective delivery of care, supporting aprogramme of equal access to mainstream services with reasonable adjustments with collaborative interventions from learning disability services.To engage with working towards service developments and embracing new initiatives, as well as supporting service outcome measures usingclinical and service audit.To contribute to the development and implementation of learning disabilities strategic planning and service development within the Learning Disability Directorate. They will participate fully in the professional activities of Nursing within the Division, gain further experience of professional issues and be familiar with current strategic thinking in the NHS in general
