Anticipated Start Date: Immediate
The principal place of work for this post is the borough of Blaenau Gwent. The post holder will generally be expected to undertake their programmed activities at their principal place of work or other locations as agreed in their job plan. Exceptions will include travelling between work sites and attending meetings away from the workplace. The post holder may be required to work at any site within the Health Board, including new sites.
Free parking is available across all Aneurin Bevan University Health Board sites.
This position involves Regulated Activity withadultsas defined by the Safeguarding Vulnerable Groups Act (amended by the Protection of Freedoms Act 2012) and the checks below will be undertaken following any conditional offer:

Main duties of the job
The post holder will be a valued member of the Frailty/Intermediate Care Directorate and responsible to the Consultant or Specialist Grade doctor for whom they are working.
The post holder will provide medical support to the borough-based Community Resource Team (CRT) as part of the multi-disciplinary team. The focus of CRT is to provide treatment or support for the older or frail person in or as close to home as possible, enabling them to remain at home, maintaining independence, and avoiding any unnecessary hospital admissions.

Job responsibilities
- The post holder will support the Rapid Medical element of the CRT, ensuring the effective and appropriate medical care of patients admitted under the care of the Consultant Geriatrician or Specialist Grade doctor
- The post holder will be expected to review patients at home or in the community setting with nurse support
- He/she will be involved in the assessment, monitoring and initiation of treatment, where appropriate, under the supervision of senior colleagues
- The post holder will be expected to participate in regular MDTs to determine ongoing treatment needs as part of the wider multi-professional team
The post holder has a general duty of care for their own health, safety and wellbeing and that of work colleagues, visitors and patients within the hospital. This statutory duty is in addition to any specific risk management or clinical governance accountability associated with the post.
The post holder is expected to:
- Observe the rules, policies, procedures and standards of Aneurin Bevan University Health Board together with all relevant statutory and professional obligations
- Observe and maintain strict confidentiality of personal information relating to patients and staff
