Rapid Frailty Response Practitioner

Job summary

The Rapid Response Team plays a vital role in admission avoidance, delivering a 'hospital at home' model of care for individuals experiencing acute illness. The team also supports early discharge from The Grange University Hospital, working closely with the Front Door Frailty Team to ensure seamless transitions from hospital to home. This role is central to supporting the frailty agenda, particularly the Six Goals for Urgent and Emergency Care, by enabling patients to remain safely at home. The post holder will work collaboratively with the integrated team at the Grange front door, using their knowledge of the community offer to support safe and sustainable discharge planning. The team operates as part of a widermulti-disciplinary network, including General Practitioners, Consultants, District Nurses, Physiotherapists, Occupational Therapists, Community Psychiatric Nurses, Social Workers, Support and Well-being Workers, and the Voluntary Sector. Key Responsibilities:

  • Deliver high-quality, person-centred nursing care in the community during periods of acute illness.
  • Undertake and contribute toComprehensive Geriatric Assessmentsto support frail individuals in remaining at home.
  • Facilitate early discharge from hospital by working closely with theGrange Front Door Teamand community services.
  • Provide timely nursing assessment, care planning, implementation, and evaluation.
  • Promote health and prevent illness through proactive interventions and education.

Main duties of the job

  • To be responsible for the assessment, planning implementation and evaluation of care for community-based patients.
  • To work as a member of the wider primary care and community multi-disciplinary team.
  • Administration of IV therapies within the community and Cedar Unit Clinic.
  • support the patients at front door at grange to be pulled back into community- with knowledge of community offer.
  • To support the Team Leader, in developing the Rapid Response Team and raising the profile of the Rapid Response Service within the Borough.
