The main duties of the posts are to care for individuals with complex health care needs in their own homes within the community.

Band 3, Health Care Support Worker
Base: Blaenau Gwent
Are you someone who has an interest working with an individual with complex health needs who wants an exciting new challenge?
Vibrant suitably experienced Health Care Support Workers are required to care for a young girl transitioning from paediatric care to adult care, living in the Blaenau Gwent Area.
HCSW should have knowledge and experience of working with patients with complex health needs, a learning disability background is desirable but not essential and HCSW's are welcomed who would be willing to learn these skills. This role would include rotation of 10 hour night shifts and a 12 hour shift within the service.
Successful staff will be expected to enable the individual to live a full and productive life supporting physical and psychosocial needs within her family unit. This unique opportunity allows the nurse to develop a relationship with the patient to know her individual needs and health needs to provide high, quality nursing care.
There is developing, exciting work within the Care at Home Team within ABUHB. The service is driven towards succession planning and there are a variety of training and development opportunities available for HCSW's within this unique service. The learning disability nursing team are at an exciting time, developing, using innovative evidence based practice to give high quality care to people with learning disabilities within the community maximising their quality of life.
