JOB SUMMARY:

This is an exciting opportunity for a highly organised and motivated individual to join The Stroke Directorate Management Team, based at The Grange University Hospital and Ysbyty Ystrad Fawr (YYF). The primary role of the post holder will be responsible for capturing, validating and monitoring data for the Sentinel Stroke National Audit Programme (SSNAP), to facilitate multidisciplinary team working and to act as a link between Management teams, Clinical Nurse Specialists (CNS), therapists and consultants. This role will also be responsible for the processing of TIA referrals in a timely manner and to ensure that they are managed in line with the required guidelines.

About SSNAP

The Sentinel Stroke National Audit Programme (SSNAP) aims to improve the quality of stroke care by measuring both the structure and processes of stroke care against evidence-based standards. These standards are informed by the National Clinical Guideline for Stroke, and national and local benchmarks.

SSNAP measures the quality of care that stroke patients receive throughout the whole care pathway up to 6 months post admission in England, Wales, and Northern Ireland. SSNAP offers a reliable and comprehensive source of data to inform quality improvements.

Main duties of the job

The post holder will be part of a busy team within the Stroke Directorate. This is a vital role that will support efficient and smoothrunning of the service, with a particular focus on SSNAP data and TIA waiting list. The role will provide administrative support toStroke team, ensuring high-quality standards.

To capture, monitor and validate Sentinel Stroke National Audit Programme (SSNAP) audit across the Health BoardTo schedule 6 monthly SSNAP assessments and monitor complianceTo manage the Transient Ischaemic Attack (TIA) waiting list, including booking patients within the current guidelines in atimely manner / send letter / undertake further referral processing to other clinicians as appropriate

To support Stroke meetings by taking notes / action points and minutesTo provide support in carrying out investigations for Stroke complaintsTo provide administrative support to Deputy Directorate Manager / Programme Manager

  • Use and knowledge of IT packages and applications, including Microsoft Office
