Job summary

We are seeking a Future Care Planning Coordinator to support in the coordination, facilitation, and training of Future Care Planning within Aneurin Bevan University Health Board Primary and Community Care. This role is essential for supporting clinical decision-making at the end of life for individuals with and without mental capacity.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

  • The post holder will have a key role in the co-ordination, facilitation and training of Future Care Planning within Primaryand Community Care within Aneurin Bevan University Health Board for those individuals with and without mental capacityto support clinical decision making at end of life.
  • The post holder will be pivotal and lead in the continued development of processes to support and embed Future CarePlanning across Primary Care, the Independent Sector and wider services across Blaenau Gwent, Caerphilly, Newport,Monmouthshire and Torfaen.
  • To analyse complex information around performance, capacity and demand data, identifying areas for collaborativeworking.
  • The post holder will possess excellent communication and leadership skills and demonstrate an ability to workautonomously as well as an effective member if the Primary & Community Care nursing team.
  • The post holder will provide highly specialist advice and act as a special resource for other health and social careprofessionals on issues relating to Future Care Planning.
