Job summary

An exciting opportunity has arisen within our Cardiac and Heart Failure rehabilitation service within ABUHB. We are looking for an enthusiastic, motivated , forward thinking individual with a love of cardiology to join our team within St Woolos Hospital. St Woolos will be the base, however cross site coverage may be required.

Main duties of the job

The main duties of the Cardiac Rehabilitation Specialist Nurse involve caring and supporting cardiac and heart failure patients who have been diagnosed with a cardiac condition, undergone cardiac interventions or surgery by providing an holistic rehabilitation programme, where health education, health promotion , modifiable risk factor management and optimisation of medical therapies are facilitated.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Job responsibilities

The Post Holder will, as part of a Multi-Disciplinary Team, assist in the facilitation of a safe and prescribed cardiac rehabilitation programme. This involves completion of cardiac risk assessments, making appropriate and timely referrals to associated agencies, working with the patient to provide risk factor modification with the aim of reducing the risk of further coronary disease, optimisation of medical therapies and education to empower cardiac patients to self manage their condition.

They will support the exercise specialists in the safe monitoring and delivery of structured exercise programs. Assist in the running of the cardiac rehabilitation service and promote continuous development in clinical practice and delivery of patient care. The post will also involve encouraging innovation and appropriate change in service delivery within the speciality of cardiac and heart failure rehabilitation in response to patient need, audit and evaluation and national guidance. To assess patients in the clinic setting and provide them with rehabilitation and health education advice. Assess their suitability for commencing the cardiac rehabilitation programme. To provide support, guidance and advice to patients and families with heart disease or heart failure. Give advice about risk factor management. To contribute to the case management of cardiac rehabilitation patients and to liaise with other members of the health care team over patient care. To facilitate input of data on ABUHB systems and the NACR audit.
