Job summary
A fantastic opportunity has arisen to work within our Cardiac Rehabilitation Service. You will be based at Ysbyty Ystrad Fawr within Aneurin Bevan University Health Board, but you'll be expected to work across the ABUHB sites as required by the service.We are looking to appoint a self-motivated and positive thinking person, with experience in cardiac rehabilitation, who can practice autonomously.This is an ideal opportunity for anyone interested in this specialist area of cardiovascular prevention and rehabilitation to develop expertise and further their professional development.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The role involves careful pre assessment for safe and effective exercise prescription; the safe and effective running of the out-patient exercise groups. You will support patients through their programme, setting clear goals for lifestyle optimisation and activity/exercise which you will review and adapt as required and at mid-point and end of programme. You will also provide ongoing goals and referrals to phase 4 community programmes following final review.You will be responsible for the care of clients during their phase 2 and phase 3 cardiac prevention and rehabilitation. You will also be responsible for entering accurate data into the National Audit for Cardiac Rehabilitation (NACR).

As part of a supportive team, you will provide and supervise a comprehensive cardiac rehabilitation programme from referral to discharge, adapted to individual clinical need and patient preference, for all cardiovascular patients meeting the referral inclusion criteria.We are looking for someone who has previous or current experience in cardiac rehabilitation with in date BACPR phase 4 instructor training.In return you will be part of a very friendly, experienced and supportive team, dedicated to helping you to develop your career in this clinical field.We encourage all staff to participate in Continuing Professional Development.
