Clinical/Practice Educator - Band 7
As the Complex Care Team continues to expand, including the Assistant Practitioner workforce within the Care at Home Team, a great opportunity has arisen for an enthusiastic and motivated individual to join this successful community based team. In Complex Care, we look after and work with individuals with significant health needs who receive Continuing NHS Health Care funding, which has seen a growth in the Assistant Practitioner role.
You will work closely with all health professionals across the health board and within Complex Care, who will also give ongoing support and guidance.
You will work with individuals, their families, nursing staff, Allied Health Professionals and Healthcare Support Workers across ABUHB in a variety of settings: including someone's home, day centres, gyms etc. You will help develop, deliver and review all training and education within the Complex Care Division. Part of your role will be giving support and guidance to members of the team. You will also act as a point of contact between the Complex Care team and others within the community, the acute sector and educational providers.
You will need to have a flexible approach, be able to work as part of a team and on your own. You must be able to problem solve.
You have to be innovative, forward thinking and dedicated to high quality care.
