Job summary

The post holder will be responsible for supporting the stepdown, discharge process and the flow of Stroke patients. You will act as the link between the Site Co-ordination hub, the Stroke ward and the Management team.
The post holder will work closely with the management, consultant, nursing and operation teams to ensure the efficient and effective management of Stroke beds to improve the tier 1 SSNAP target of ensuring that patients reach the Hyper Acute Stroke Unit within four hours of presentation to the Emergency Department.
This is a fixed term post for 6 months to meet the needs of the service
Main duties of the job

The post holder will work in collaboration with other Healthcare Professionals to ensure safe and appropriate utilisation of Stroke beds, as well as co-ordinating safe transfers and discharges.
The post holder will also support the SSNAP co-ordinator with data collection and input into necessary systems to ensure accurate reporting of KPI's across the Stroke pathway.
To support the discharge process of Stroke patients
To act as a link between the operations team and the Stroke wards based at the Grange University and Ysbyty Ystrad Fawr hospitals
To co-ordinate bed plans and discharges
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
