Job summary
Monmouthshire North Neighbourhood Care Network (NCN) has an exciting opportunity for a Practice Pharmacist to work with GP practices in the area. The role includes working in a clinical role in GP practices to meet the health needs of the NCN population. You will work across a number of practices in North Monmouthshire, interacting with a range of healthcare professionals. There is professional support and engagement with the ABUHB medicines management team.
Main duties of the job
You will be responsible for delivering a pharmacy service to GP practices within North Monmouthshire NCN area and a range of tasks (not exhaustive) including: Being responsible for developing and managing systematic medication and polypharmacy reviews, which will take into account social and practical issues for the patient as well as clinical elements.
Developingpharmaceutical care plans for individual patients as appropriate, undertake medicines reconciliation following admission or discharge and patient moving into/out of care homes and those newly registered with the practice, following up medication related issues with the patient or other professionals.
Working autonomously where appropriate to meet the needs of the patients and to contribute to the delivery of NCN and practice delivery plans.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
