An exciting opportunity has arrived for an enthusiastic individual who wishes to support the public in making healthy lifestyle to play a key part in the role out of the All Wales Diabetes Programme across ABUHB.

Our vision is to provide more care closer to home with primary care and community services at the heart of what we do to support patients to prevent ill health and maintain and improve their own health and well being.This is an exciting opportunity for a healthcare support worker with a passion for helping others, with good communication skills and a keen interest in nutrition and health.

The post holder will perform brief intervention sessions as created by the All Wales Diabetes Prevention Team within GP surgeries. They will support patient requiring lifestyle support after their chronic condition reviews & carry a delegated caseload of patients to support with healthy lifestyle changes. They will be expected to liaise with a number of stakeholders and co-ordinate patients care.

The post holder will support the work of registered dietitians as well as other clinicians in the primary care teams and would be required to work within the British Dietetic Association (BDA) Professional Code of Conduct for dietetic assistants.

Main duties of the job

You will receive nutrition and communication skills training to deliver brief intervention with patients identified as being at risk of having diabetes. This will involve having diet and lifestyle conversations and signposting to local services to support a reduction in the risk of development of diabetes. The role will include the collection of a minimum data set and support of the development and distribution of written resources. Administrative duties and good IT skills will also be an essential requirement of the post.

The post holder will play a vital role in participating in the all Wales Diabetes Prevention programme which is being rolled out across Wales. You will have support and supervision from a senior dietitian and links to the network across Wales.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
