Job summary

The Dermatology Team based at St Woolos Hosptial are looking for a motivated and enthusiastic registered nurse to join their team.

We are based at St Woolos Hospital but provide services to the population of Gwent with clinics held in Royal Gwent, Nevill Hall and Ystrad Mynach. The Clinical Nurse Specialist role is varied. The team are responsible for education of patients and monitoring of those on systemic therapy. Improving links with primary care for those with chronic skin disease.

Main duties of the job

  • To competently provide specialist dermatology nursing knowledge, clinical skill, support and advice for patients with skin disease, their families and carers across the Dermatology Directorate and hospital sites
  • To work within the Dermatology team, primary healthcare, families and carers to provide a co-ordinated approach, ensuring continuity of care for patients.
  • To be the key worker throughout the treatment phase of the patients journey from initial diagnosis, through treatment and follow-up as required in the nurse-led clinics
  • To act as a resource to and participate in effective educational activities in order to support competent clinical practice for all healthcare professionals involved in Dermatology care.
  • To undertake protocol driven, nurse-led clinics, which compliment the outpatient service as directed by the senior nurse /Lead Consultant Dermatologist for the health board.

Principle Responsibilities
  • Clinical Practice
  • Provide specialist care for patients with inflammatory skin disease recognising the needs of the individual and providing support, information, education and counselling for patients and their families.

1.2 Co-ordinate care for patients with skin disease, liaising with the Multi-Disciplinary Team.

1.3 Giving, clarifying and reinforcing information over time which is sensitive to the patients level of understanding, language and culture.

1.4 Maintain accurate documentation and collate statistical information as required.

1.5Assist in developing clinical guidelines and protocols jointly with the Dermatology team.

1.6 Participate in the development and distribution of patient information, ensuring it is accurate, appropriate and timely.

1.7 Attend nurse-led clinics and autonomously mange own caseload of patients with chronic inflammatory skin disease; providing topical and systemic treatment plans, drug monitoring and evaluation of plans of care in line with local and national guidelines

1.8 Assist patients as required in outpatient clinics to allow of whole skin examination if appropriate for disease assessment

1.9 Obtain samples for diagnosis/monitoring if required; skin swabs, skin scrapings, nail clippings, urine samples

1.10 Work independently as well as working with and supporting others in the team

  • Discuss complex dermatology management plans with patients and medical staff
  • To have an allocated case load of patients
  • To provide advice to patients and carers via the Dermatology CNS advice line
  • To be responsible for monitoring the disease process and outcome, including monitoring drugs with appropriate clinical and laboratory parameters in line with local/national guidelines
2 Management/Leadership
  • Liaise with the Senior nurse for Dermatology and lead Dermatology consultant to ensure caseload is managed
  • Develop good working relationships with the multidisciplinary team and attend formal and informal meetings as necessary
  • Establish and maintain effective working relationships with staff throughout the Health board and all other agencies, both voluntary and statutory.

  • To provide professional advice and support colleagues and all agencies involved with dermatology patients.

2.5 Maintain personal and professional development, which is commensurate within the Dermatology service.

  • Develop with the multidisciplinary team care pathways for dermatology patients.
  • Work in partnership across primary and secondary interface to identify ways of improving services and reducing inpatient admissions

Contribute to the operational and strategic development of dermatology services across the health board including engaging in change management

  • Participate in planning a wide range of formal and informal teaching sessions to include staff, patients and carers
