Job summary

An exciting opportunity as arose for a experienced Nurse with an advanced level of skills and experience to join the South Monmouthshire CMHT based at Hywel Dda Chepstow. The CMHT works with Adults with serious and enduring mental health conditional living in the south Monmouthshire area.

Successful candidate would be expected to have advanced skills and experience of working with individual with complex needs. Be trained to a high standard that may include medical and psychological interventions. As well as providing clinical supervision and leadership of nursing staff within the team.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Main duties of the job

  • be committed to providing excellence in Person Centred Care that will include the promotion of recovery, social inclusion and independence for individuals for whom they provide care and support.
  • provide individual clinical intervention within an evidenced based person centred pathway and will ensure effective co-ordination of care across intermediate care services to ensure the required pathway of care is followed at the right time to meet patient needs.
  • develop the specialist role within the team and is expected to provide specialist knowledge, identify training needs and provide team training in relation to Mental Health.
  • be expected to provide clinical leadership and management to enable the development and provision of services where co-morbidity is a key issue.
  • undertake an extended clinical and nursing role to support nursing, medical and support staff in providing high quality patient care for people within their own homes, ensuring the maintenance of clinical excellence.
  • Make professional autonomous decisions, for which he or she is accountable and act in accordance with the NMC Professional Code of Conduct and other relevant policies.
  • work collaboratively with and support members of the multidisciplinary team in the delivery, development and management of complex health and social care issues.
