Job summary
An exciting opportunity has arisen for a Band 4 Assistant Practitioner to join the Caldicot Community Team on a permanent contract for 31.5 hrs (part time) over five days a week, including some weekends.
The Caldicot team provide clinical care and interventions for patients in their own home under the medical governance of Primary Care GP's.
The District Nursing/Community team in Caldicot works as a partner within a wider multi-disciplinary team which includes CRT Frailty Consultant, General Practitioners, Care at home, Physiotherapists, Occupational Therapists, Social Workers, Support and Wellbeing workers and the Voluntary Sector.

An exciting opportunity has arisen for a suitably qualified Band 4 Assistant Practitioner to join the Caldicot community team in the provision of clinical care to individuals in their own home.
As Assistant Practitioner you will work with the wider community MDT under the delegation of your Band 7, Band 6, CCN and a team of Registered Nurses. You will utilise and develop a wide variety of skills to meet the emotional, physiological, and clinical needs of individuals.
The post holder will work collaboratively with and support members of the multidisciplinary team in the delivery, development and management of health and appropriate referral for social care issues. The role is challenging but rewarding and you will be fully encouraged and supported with your professional development.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
