Job summary

An opportunity has arisen within the Caerphilly Adult Mental Health Service to appoint to a Community Psychiatric Nurse post in the South Caerphilly area. The post holder will be a senior member of the South Caerphilly Community Mental Health Team and will be responsible for the care coordination of a caseload of service users who are experiencing severe mental health difficulties, and will have additional complex care needs.

Main duties of the job

The successful candidate will work collaboratively across the wide ranging multi-disciplinary services and agencies to provide effective delivery of health and social care, supporting a programme of recovery and social inclusion within the area. The post holder will also, as part of the staff resource in the duty desk, support the delivery of timely and effective intervention of all individuals who are in need of urgent mental health assessment and management.

The post holder will be expected to develop close working relationships with the other aspects of the service both community and Inpatient Unit, as well as develop a broad knowledge of the resources available across the Borough. They will be expected to support the Team Leader to achieve organisational targets. The successful candidate will have excellent communication and interpersonal skills and have a proven track record of working with service users with complex mental health needs. They will have a commitment to providing person centred care and will, through their actions at all times demonstrate a strong value base and a preparedness to embrace change.
