Job summary
We are recruiting into an established district nursing team within the borough of Torfaen. The post holder will be part of the team who provide a diverse service to our community house bound population.
The team comprises of a team manager and team leader. The team have full time and part time band 5 nurses and band 4 and 3 HCSW's. There is full time administrative support.
The team work in collaboration with the aligned GP surgeries.
Main duties of the job
To provide assessment, planning, treatment and evaluation to patients on the district nursing caseload and admission of new patients.
Clinical skills in those areas required for the role, the successful candidate will be supported with clinical competencies in addition to offering career development.
To work collaboratively with other MDT professionals and to GP surgeries aligned with the caseload.
To work within the team as well as working alone which is supported by the lone worker policy.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
