Job summary

An exciting opportunity has arisen to join Newport Community Mental Health Team as a crisis triage practitioner.

Main duties of the job

The role is exciting and dynamic and involves being the first point of contact in the Community Mental Health Team for referrers and patients via telephone and email, it requires excellent interpersonal skills. The successful candidate will need to be highly skilled in assessing mental state and risk, they will take responsibility for screening referrals, whilst determining the urgency. The role will also involve working with an existing patient group who are familiar with the service and require on times additional intensive case management to avert a further escalation of their mental health.

The role involves developing collaborative plans and consultation with the wider CMHT, crisis liaison Home treatment team and primary care mental health support services.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
