The main duties of the posts are to care for individuals with complex health care needs in their own homes within the community.

Main duties of the job

An exciting opportunity has arisen for a band 8a Complex and Long-Term Care Pharmacist to work as part of our Primary Care Pharmacy team. You will work with Care Homes, supporting the Complex and Long-Term Care across Division Aneurin Bevan University Health Board.

The role includes:

  • Managing and developing the Complex and Long-Term Pharmacy team working across the Health Board geographical area
  • Improving patient safety through ensuring effective medicines management in care homes, independent hospitals and the Divisions Community Teams, working in partnership with all appropriate agencies and staff
  • Close partnership working with Primary Care, the Divisions Community teams and the Providers to manage the excessive prescribing and ordering of medication.
  • Lead on and work with colleagues to establish and develop good medicines management systems across the Divisions Community Teams home/independent sector, promote evidence-based, cost-effective prescribing and support ABUHB initiatives to promote and improve patient self-care
  • Provide ongoing pharmaceutical expert advice to care homes /independent hospitals and Divisions Community Teams
  • Actively working with general practice and community pharmacy to develop efficient repeat prescribing / dispensing systems to support the Divisions Community Teams, care home prescribing across the whole sector and reduce waste
