Main duties of the job
An exciting opportunity has arisen for a band 8a Complex and Long-Term Care Pharmacist to work as part of our Primary Care Pharmacy team. You will work with Care Homes, supporting the Complex and Long-Term Care across Division Aneurin Bevan University Health Board.
The role includes:
- Managing and developing the Complex and Long-Term Pharmacy team working across the Health Board geographical area
- Improving patient safety through ensuring effective medicines management in care homes, independent hospitals and the Divisions Community Teams, working in partnership with all appropriate agencies and staff
- Close partnership working with Primary Care, the Divisions Community teams and the Providers to manage the excessive prescribing and ordering of medication.
- Lead on and work with colleagues to establish and develop good medicines management systems across the Divisions Community Teams home/independent sector, promote evidence-based, cost-effective prescribing and support ABUHB initiatives to promote and improve patient self-care
- Provide ongoing pharmaceutical expert advice to care homes /independent hospitals and Divisions Community Teams
- Actively working with general practice and community pharmacy to develop efficient repeat prescribing / dispensing systems to support the Divisions Community Teams, care home prescribing across the whole sector and reduce waste
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
