Main duties of the job

ROLE SUMMARYThe post holder will ensure the provision of a service for individuals with mental health needs, as part of an integrated service at Monnow Vale. The post holder will undertake an extended clinical and nursing role to support all staff within Monnow Vale ensuring the provision of high quality care. A key part of this role is working in the memory Assessment Service.

The post holder will be a member of the Integrated Services Team based at Monnow Vale. They will be responsible for the care coordination and treatment/intervention of service users who receive care from Monnow Vale and who have mental health needs.The post holder will take responsibility within their caseload for ensuring the assessment of health and social needs, formulation, planning, delivery and evaluation of care, support and treatment where appropriate by the team, for both individuals and groups of service users including risk assessment and management, CTP and care reviews. They will work collaboratively across a wide ranging multi-disciplinary services and agencies to provide effective delivery of health and social care, supporting a programme of recovery and social inclusion within the area.
