Job summary
An exciting opportunity has arisen for a full time Band 6 Community Nurse, Integrated Assertive Outreach Team (AOT) in Caerphilly, the successful applicant needs to be highly motivated, innovative and enthusiastic about working with service users in the community within the Community Assertive Outreach Team based in Hafan Coed, Bloomfield Road, Blackwood.
Main duties of the job
This is an excellent opportunity to be part of an established and evolving integrated AOT. You will have knowledge and experience in the care of people with complex, severe and enduring mental health problems in a community setting. You will have the opportunity to link with the wider service which includes the CMHT clinics, CRHTT, and the inpatient ward. The AOT provides a specialist service offering comprehensive assessment, treatment and care of service users who are experiencing severe mental health difficulties within the local community.
The post holder will ensure the comprehensive provision of a cost effective, accessible, appropriate and responsive mental health service to service users with complex mental health needs in the Assertive Outreach Team. They will be responsible for the implementation of the Mental Health and Recovery and Rehabilitation strategy within the service and develop policies and protocols to support the associated principles and values.
The post holder will carry the responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance and provide specialist knowledge and management around the risks of patients. The successful applicant will develop and coordinate a multi-professional approach providing evidence based mental health care to all those who use the service.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
