Pharmacist (Primary Care) - Brynderwen & Minster Surgery

Job summary

The post holder must be a registered pharmacist with the General Pharmaceutical Council (GPHC) who may also have the additional qualification of Independent Prescriber and who must act within their professional boundaries. This is a new and developing role with the responsibility for supporting the Partners in implementing effective medicines management within the Practice; identifying areas for improvement, initiating and managing change. The post holder will provide care for patients in all aspects of medicines management and, who should demonstrate safe, clinical decision-making and expert care for patients. The post holder will undertake clinical medication reviews to proactively manage patients including those with complex polypharmacy and comorbidities. The post holder will provide primary support to the wider team with regards to prescription and medication queries, helping support and develop the prescription pathway and processes within the surgery. This will include dealing with acute and repeat requests, medicine reconciliation on transfer of care, providing expert advice in clinical medicine whilst remaining conscious of public and social care needs of patients. The post holder will provide leadership on medicines optimisation and quality improvement. 

The successful candidate will be employed by the GP Practice/Medical Centre, not Cardiff & Vale University Health Board

Main duties of the job

We are looking to recruit into a new, exiting role within the practice for a driven General Practice Clinical Pharmacist to support us in developing and integrating the clinical skills of a pharmacist into the Primary Care Team. You should be passionate, enthusiastic, motivated and, committed to developing your knowledge and skills; working together with the team to deliver the best possible care we can to our 19,000 patients. This is an exciting opportunity in a busy environment.

You will be part of a friendly, innovative, growing team of receptionists, doctors, nurses and administrative staff working collaboratively with allied colleagues across multiple disciplines and agencies, within the driven, harmonious Cluster of Cardiff East. As a training practice we place great emphasis on teaching and ensuring that we pay forward our knowledge.

Full training and support will be provided with a structured induction period to ensure a full understanding of the VISION clinical system; you should have sound, general computer knowledge. Flexibility in working hours is necessary to cover any periods of colleague absence.

The practice serves a diverse population of circa 19,000 patients across East Cardiff.The practice has two sites, Brynderwen Surgery in St. Mellons and Minster Surgery in Roath.The practice strives to deliver excellent, evidence based care through a team who work together in harmony. We aim to be a happy and pleasant working environment with a diverse team of clinicians and administrators.

The post holder will develop, and support team members involved in the practice prescribing processes including clinical supervision, training and leadership

The post holder will be required to be responsible, alongside all clinicians, for some aspects of contractual requirements (QAIF, Enhanced Services etc.).

This is a permanent contract for 6 sessions per week (1 session = ½ a day) with a negotiable salary, dependent on experience.
