Job summary
This is an exciting opportunity to join either the hospital Palliative Care team or the Supportive Care team which covers both the hospital and community. The hospital Palliative role is looking for an indiviudal to work alongside the registered Palliative Care Occupational Therapist to provide inpatient care and assessment to patients approaching end of life. To assist the patient to gain maximum function with the aim to discharge home in a timely manner. The Supportive Care team provide care to patients with cardiac, renal, liver or respiratory conditions during the last 1 - 2 years of life. The aim is to reduce hospital admission, reduce symptom burden and improve quality of life. Care is provided in the hospital, clinic or community across Cardiff and Vale. The ability to travel across Cardiff and Vale to review patients in their home under the guidance of the Allied Health Professional lead - Physiotherapist, Occupational Therapist or Dietitian. Both roles will be extremely rewarding however can be challenging supporting patients towards or at the end of life. The team works closely with community services and the hospital and community palliative care teams. Excellent communication skill is vital and the ability to quickly adapt to the patients changing condition.
As an AHP Technician the post holder is expected to independently manage their own work and case load, undertaking tasks delegated by a registered practitioner with appropriate supervision in place.
Main duties of the job
To provide a service with the Therapists that will promote physical andfunctional rehabilitation and the nutritional status of patients.To work under supervision and assist the Therapist in clinical andadministrative duties.To be delegated routine interventions to be carried out independently, andto progress these interventions within guidelines laid down by the Therapists.In addition, the post holder may be required to work in other areasdepending on service needs.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
