Do you want to join an exciting new team? We have developed a planned care Pre-Operative pathway for frail patients through the Perioperative care of Older Patients undergoing Surgery(POPS) team. The aim of this team is to review patients age >=65 years who are living with frailty using Comprehensive Geriatric Assessment, upskill staff in geriatric syndromes (eg frailty, delirium) plus interact with therapies teams to name just a few! Central to this is engaging in shared decision-making models of care.
Experience
- Sufficient post registration experience, ideally with a surgical background.
- Embed the core NMC values/expectations of care, compassions, courage, communication, commitment, and competence into everyday practice.
Skills and Knowledge
- Ability to motivate others by influencing and inspire others in new enhanced ways of working
- Strong leadership and a shared vision for better care
- Highly developed interpersonal skills, negotiation, conflict management, feedback, partnership working, and coaching skills
- Able to develop effective and mutually supportive relationships with key partners
- Strong intellectual, strategic, and systemic thinking skills, with the ability to think creatively and laterally to achieve outcomes
- Strong IT Skills.
- Current qualification/nurse registration with NMC & demonstration of CPD
Main duties of the job
An exciting opportunity has arisen for a Registered Nurse to join the multi-disciplinary Perioperative care of Older Patients undergoing Surgery (POPS) team based within the Pre-Operative Assessment Clinic.
The duties and responsibilities of the Elective Surgery Frailty CNS are to:
- Offer clinical leadership and direction to members of the multi-disciplinary team.
- Have experience of working with frailty /older population demographic.
- Base care plans on the principles and domains of a Comprehensive Geriatric Assessment, including assessment of the physical, psychological, functional, social and environmental needs of the patient
- Make independent clinical decisions/judgements based on patients' clinical needs, request/ order investigations/ tests, provide, review and an action plan following.
- Identify and/or engage in locally organised shared learning opportunities as appropriate and as capacity allows
The post is offered full time, 37.5 hours per week and will operate between the hours of 0800-1800 Monday to Friday (actual hours to be agreed with postholder)
The ability to work flexibly and support the development and growth of the role/service is essential.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
