Job summary
The Assistant Practitioner is responsible for supporting clinical staff to progress the patient's journey in a structured and timely way through the patient pathway. This will occur through a process of tracking or navigating various aspects of patient care, and will require effective communication with other clinical and non-clinical departments.
The Assistant Practitioner observes patient condition, records clinical observations and reports to a registered nurse. They contribute to assessment of care needs and carries out nursing procedures e.g. skin prick testing, takes blood samples, collection of blood products.
The Assistant Practitioner will have a crucial role in liaising with home care delivery providers and the safe and precise monitoring of patients home therapy including blood product batch recording.
Main duties of the job
Clinical Duties
To manage, under indirect supervision, own workload within clinic area and competencies of the post.Under the direction of the Immunology nurses, maintain the Immunoglobulin Register.
Prepare documentation for pre-treatment, immunodeficiency and immunotherapy clinics.
Provide a comprehensive administrative service and review provision in order that improvements and developments can be made to the work undertaken.
Present patient information and data at internal and external meetings, for example, at multi-disciplinary meetings.
Document all Immunology Nurse Specialist activity and relevant patient monitoring data on local, regional and national databases and PMS as required.
Assisting Immunology Nurses in the treatment pathways and organisation of patients attending clinic.
To undertake venepuncture, cannulation and skin prick testing as directed by the relevant clinicians and prompt processing of blood samples.
To be responsible for obtaining the results of tests and investigations and ensuring that the requesting clinician is informed of the results promptly.
To record: progress of immunology and allergy patients as they proceed through the treatment pathways. Responsible for maintaining and validating accurate and up-to-date immunology patients treatment databases including liaising with all spoke centres across Wales.
To be proficient actively in the understanding of all triage telephone enquiries and advice following guidelines for patients.
