Job summary
This is an exciting opportunity for an enthusiastic and dynamic pharmacistto lead the development of pharmaceutical service provision and to provide expertise to the emerging model of Enhanced Community Care.
This is a new role within the team and as the successful individual, workacross different Health Board teams, creating and managing a dedicated pharmacy resource to the community, facilitating reablement, working with primary care contractors and Local Authorities to create and manage pharmaceutical expertise to their services to ensure appropriate medicines governance and use in the community, including domiciliary care settings and care homes.
We are looking for an individual who is an experienced Clinical Pharmacist, with experience of Primary and Community care, managing change, developing and implementing policy and procedure, and provision of training. You will be an excellent communicator with strong interpersonal and relationship building skills and the ability to influence. You will also have excellent knowledge of quality improvement, safety in medicines use and promoting the pharmacy professions.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply
Main duties of the job
Provide strategic oversight and assurance of governance around medicines use and prescribing in the community, supporting the development of the evolving service model
Risk asses the care pathway relating to medicines use and prescribing to ensure that prescribing, medicines supply and medicines use aligns with National and local guidance and legislation.
Provide support and mentorship to all pharmacists and pharmacy technicians providing care to our frail population in the community
Provide expert clinical pharmaceutical care to our frail population - deprescribing and optimising medicines use , and supporting other healthcare professionals to do the same.
Assess, recommend and support implementation of solutions to improve the transfer of information relating to medicines.
Analyse and interpret the prescribing data for community services enabling the monitoring and improvement of safe and cost-effective medicines use.
Operationally manage the pharmacy team input to community beds and other community teams
Review clinical incidents and promote learning to improve medicines use and prevent avoidable harm from medicines in the community
Undertake audit and evaluation of the benefit of the pharmacy professional resource within community teams, build the team and support as demonstrated by the need
