Job summary
Additional funds have been released by Welsh Government to support with the delivery of the 'All Wales Dementia Pathway of Standards'. With this the Cardiff and Vale Memory Assessment Service are looking to appoint 2 further Memory Link Workers into their established multidisciplinary team. The Link Worker role provides post diagnostic support to people and their carers living with dementia, acting as the named contact from point of diagnosis to end of life. The aim of the role is to support people and their carers living with dementia to live as well as possible through being the point of contact for signposting and navigation of services.
Main duties of the job
The link worker roles are aligned to the General Practitioner Clusters, and it is anticipated that these new posts will be allocated to regions where there are disproportionately low dementia diagnosis rates given the population risk factors. For Cardiff, these areas have diverse demographics and are densely populated with Ethnic Minority groups.
The role requires individuals to be actively involved in the mapping and making connections with statutory and third sector community services within their Cluster, and promotion of their service within the Cardiff and Vale Partnership Board.
The successful applicants will ensure that the support provided is Person-Centred and guided by having 'What Matter's' conversations with people. It is a highly challenging role, but one that has huge rewards that can have a significant impact on the quality of life for the people they support.
