Job summary

The renal nurse specialist is an integral part of the multi disciplinary team, providing nursing expertise and professional leadership within inpatient, outpatient and community and shared care in relation to the treatment management and nursing care of children and families with chronic Kidney disease.

This is a unique opportunity for a passionate and experienced Renal Nurse to join a close-knit team to provide care in the community for a caseload of pre-dialysis and peritoneal dialysis patients.

Excellent communication and organisational skills are essential to this role. You will need to demonstrate a practical, forward thinking approach to service development and evaluation and the ability to work well within a team.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

The role includes managing a caseload of patients, education of patients, their families, staff in the community, inpatient and outpatient settings. To actas a point of contact for families, multidisciplinary team, other hospitals, other agencies in the community for specialist advice and information.

The successful candidate will be expected to:

  • Assess, develop and implement specialist renal nursing care programmes, provide advice, education, support and care of children and families with chronic / end stage kidney disease in the community setting.
  • Preparation and education of child and family , home assessment training for home PD , school and nursery visits
  • Provide ongoing emotional and psychological support to patients and their families.
  • Participate in and evaluate research and clinical audit in relation to specialty to inform and improve future practice
  • The post holder will support families with charity applications/grants
  • Work with the new National Paediatric Renal Nursing Competencies developed to address challenges faced by paediatric renal units.
  • Participate in youth work program workshops.

The clinical nurse specialist will also be expected to support renal outpatients and haemodialysis depending on service demands, training will be provided. Once trained the right candidate may be expected to support the Haemodialysis on call
