Job summary
The role will operate within a highly specialised Social Work Service, which includes assessment and psycho-social interventions for patients and their families/carers who attend the Paediatric Oncology Unit.
The successful candidate(s) will work autonomously and independently assessing, formulating and implementing support plans for patients and their families/carers within the Paediatric Oncology Unit. The post-holder will be responsible for liaising with external agencies to help patients and their families/carers to access appropriate support/services.
They will play a key role in sustaining strong and positive relationships with a range of internal and external stakeholders and support the delivery of LATCH's grants provision for families/carers. The post-holder will be expected to work in partnership with patients and families/carers, identifying and utilising strengths and working closely with other agencies and voluntary groups to offer practical support.
They will promote high professional standards, working within the Code of Professional Practice for Social Care at all times, upholding and embedding quality and safety across their work.
Main duties of the job
The post holder will:
- Contribute to the development, implementation and monitoring of continuous improvement initiatives.
- Participate in and promote a culture of a learning organisation, across the UHB and LATCH.
- Ensure that quality standards are met, maintaining a consistently high standard of patient care and ensuring the families/carers are at the centre of all initiatives.
- Demonstrate integrity and compassion in their work and show enthusiasm for continuous service quality improvement.
- Work in close collaboration with UHB departments, LATCH Social Work colleagues, and other internal and external stakeholders to ensure high standards are met across the UHB and LATCH.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
