Job summary

We are looking for an experienced pharmacist with the expertise and drive to deliver and strategically develop the Pharmacy Intestinal Failure (IF) and Parenteral Nutrition PN Service. As the lead pharmacist for the multidisciplinary PN Team you will deliver the pharmaceutical expertise to the Tertiary Specialist Intestinal Failure and Home PN service. You will lead the Pharmacy Nutrition team, providing managerial and clinical leadership. You will be responsible for the delivery of patient-orientated, non-medical prescribing and pharmacy services for IF and PN inpatients, home PN patients and outpatient clinics.

You will practice as an expert/consultant pharmacist and maintain connections and clinical engagement with wider multidisciplinary teams. Engaging with a network of specialist pharmacists across UK, you will champion learning, and capability development to prepare for future demands.

You will have excellent communication and interpersonal skills, decision-making and specialist transformational skills with evidence of recent service development.

Main duties of the job

  • Practice as an expert/consultant pharmacist in IF and Parenteral Nutrition PN.
  • Lead the pharmacy service for IF and PN and act as an expert resource for this area of practice
  • You will provide managerial and clinical leadership to the pharmacy team providing IF and PN services.
  • Work with relevant stakeholders, including WHSSC, to develop and agree models of care that optimise the pharmaceutical care for IF and PN patients across the UHB and Tertiary services.
  • Work closely with Pharmacy Aseptic Services to optimise the preparation service and provision of PN across the UHB and HPN service.
  • Share best practice in IF and PN care across Wales through collaborative working,
  • Continue to develop quality standards for patient-facing pharmacy services, supporting performance measures and strategies to benchmark clinical contributions.
  • Lead education and development in Parenteral Nutrition for pharmacy in CAVUHB and input to the related local governance agenda.
  • Identify and participate in the development of an All Wales strategy for Research and Development in clinical pharmacy for Parenteral Nutrition; contribute to and undertake a research programme based on the strategy.
  • Formal performance review and objectives setting will occur annually with the Line manager and Consultant Gastroenterologist and lead for the HPN service
