An exciting opportunity for an advocator and practitioner in Dementia Care Mapping to be involved as a clinical lead for this service development within MHSOP.
The Post Holder will develop and then enable Dementia Care Mapping as a person centred dementia approach process within health, social and third sectors across Cardiff and the Vale of Glamorgan. Dementia Care Mapping will support the Dementia Learning and Development Team to promote person centred care to be embedded within practice and both Teams will be required to extensively collaborate and work together.
You will be responsible as a designated lead for supporting the strategic development of the DCM service and provide the clinical leadership for the role out of DCM across Cardiff and Vale of Glamorgan by initiating and overseeing a rolling programme of DCM, for monitoring activity data and undertaking audit and evaluation.
You will be responsible for understanding the ongoing professional developmental needs of DCM staff in line with the Good Work Framework and develop a strategy for DCM practice and staff development.
You will have clinical, procedural and financial management responsibility for the DCM Team once this is established and provide operational line management and support to DCM team staff including caseload supervision.
Skills emphasis: advanced practice in Dementia Care Mapping, leadership and management, service development.
Main duties of the job
