Job summary

The role is to support and deliver the provision of health care & positive mental health promotion to the people of Cardiff & the Vale at the primary care level. The post holder will work as part of a multi-disciplinary, multi-agency Local Primary Mental Health Support Service (LPMHSS) with specific responsibility for screening, assessment, liaison, and interventions.

Main duties of the job

The aim of the PMHSS is to provide services in primary care for patients experiencing mild to moderate or stable severe and enduring mental health issues. Other service functions include support, education, and liaison for primary care team members. The main functions of the role are; consultation and advice to the primary care team; liaison with secondary mental health services, third sector services, and service user-led organisations; mental health screening & assessment; short term therapeutic work including group and one to one work; and where appropriate, signposting and onward referral to appropriate services.
