Job summary

An exciting opportunity has arisen to join our recently opened Integrated Assessment and Care Unit (IACU), providing 55 beds, at the Lakeside Building on UHW site.

IACU will provide a proactive, co-ordinated assessment unit for patients who require complex discharge planning leading to more supported and more timely manner discharge.

The unit is led by an enthusiastic Multidisciplinary Team which focusses on providing excellent, proactive care and have knowledge and expertise in discharge arrangements. The Ward Team is supported by an Integrated Discharge Team, who are also based in the IACU.

You will have the opportunity to create an active reablement environment to ensure that people are able to transfer out of hospital as quickly as possible

Main duties of the job

To be responsible for the assessment of care needs

Develop, implement and evaluate care plans

To be responsible for own group of patients

To advocate on behalf of patients to ensure the patient's voice is heard

The successful candidate will work closely with the multidisciplinary team

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
