A unique opportunity has arisen for an experienced Occupational therapist to join the Wellbeing Matters service. The main responsibilities for this role are to set up and coordinate a single point of access for community Health board equipment ordering. With the aid of an OTA you will be responsible for the assessment of incoming referrals to ensure patients needs are met first time. This will be primarily fast track bed ordering for palliative patients or to ensure their safe and timely discharge from hospital.
You will be responsible for the Line management of an Occupational therapy assistant.
This post will be part of a community triage service which is being developed to work across health , social and 3rd sector broker organization's to create a responsive service to aid patients and citizens.
Assessing patients for onward referral to suitable organisations such as the Vale community resource service or social work and facilitating step up / down facilities will be a major part of this role.
Main duties of the job
o To set up and coordinate a Single point of access for community emergency bed referrals.
o To be the Single point of contact for community health board equipment ordering.
o Be responsible for the assessment of incoming referrals to ensure patient needs are met first time. Assessing any additional patient needs.
o Be responsible for the step up and step down of community beds and equipment.
o Oversee and allocate work for an Occupational Therapy Technician.
o Monitor a caseload of patients at home with long term conditions ensuring their equipment needs are met. Reassessment at regular intervals.
o Set up systems and procedures to enable to timely response to all referrals facilitating prompt hospital discharge.
o Set up and hold a database of community equipment to ensure repurposing where appropriate.
o Liaising directly with Joint equipment stores (JES) and local authority Occupational Therapists to ensure a smooth transition between Health and Local authority funding.
o Liaise with tissue viability specialist nurse (VN) /team to ensure real time Knowledge of stocks of equipment available.
o Improve the efficiency of the approval and delivery process in conjunction with the TVN and JES.
o Help to maintain citizens with Health and Social Care needs in the community

To provide leadership through supervision, induction, training and education of Band 5 Occupational Therapists, support staff and volunteers according to organisational policy and accepted professional standards.
