Job summary

As part of a Primary Care Sustainability Project, a successful pilot led to a new and innovative Primary Care Liaison Service. We continue to develop and have opportunities to work in our primary care service in Cardiff and/or the Vale of Glamorgan and the Universities across Cardiff and the Vale . You will be mental health professionals available to support the work of General Practitioners and other inhouse practitioners, providing expert advice to patient and primary care practitioner, assessment, referral on to appropriate services, signposting, follow-up reviews, and mental health first aid. You will be instrumental in supporting patients much earlier in their pathway to improve patient experience and access to specialist support closer to patients' homes in line with Care Aims. We are looking for a further 2.4 Practitioners to join our 17 other team members to provide care to all 9 of the clusters of GP Practices in Cardiff and Vale. We have also developed a new service that is based in the Universities of Cardiff. In addition to the Liaison Service, the UHB have enhanced 3rd Sector support to provide Tier Zero and Guided Self Help (CCI) interventions within each Cluster, as a direct support to the Primary Care Liaison Service.

Main duties of the job

The PCLS is looking for 2 FT staff and one PT staff to continue the roll out of this ground-breaking service . Role consists of assessment/ triage within GP surgeries to support General Practitioners. You will also liaise with other services across CAV or in the 3rd sector to ensure seamless care. There is an ability to 'hybrid work' offering remote working and laptop/ phone. The successful candidates must demonstrate in-depth knowledge of Community Mental Health Practice, an up-to-date knowledge of the delivery of psychological therapies and support. We work with the Cardiff universities offering placements to future nurses. We have opportunities within the team allowing progression within the service, with supervisions and clinical supervision to allow progressive practice. Training opportunities are encouraged. We work within a multidisciplinary approach and welcome applications from Registered Mental Health Practitioners from Nursing, Occupational Therapy or Social work. We will consider applications from experienced band 5 staff.

This new service forms part of the Psychology & Psychological Therapies Directorate, which is currently based at the Cardiff Royal Infirmary Building. The successful candidates will be based in a practice(s) and appointments/bases will be guided by a project implementation plan. You can expect to receive excellent access to clinical supervision, peer support and further study to enhance career development.
