Job summary

To deliver evidence based holistic care to clients within the context of a multidisciplinary, multi-agency Community Mental Health Service.

To take responsibility for a clinical caseload providing comprehensive assessment, care and treatment to clients referred to the Community Mental Health Team in accordance with Parts 2 and 3 of the Mental Health (Wales) Measure 2010.

Foster robust working relationships with all partners and key stakeholders, in particular Local Authority, Primary Care and the Third sector.

Contribute to the continuing improvement and development of community mental health services.

Main duties of the job

An exciting opportunity has arisen to join the South and East Cardiff Locality Community Mental Health Service. We are currently developing the Locality Model so though initially based in the Hamadryad centre in future this post may be extended to the wider South and East locality

The team work with a diverse range of clients within a supportive team environment. The post holder will need good risk management and decision making skills together with the ability to work under pressure. You will be part of a close multi-disciplinary teams in an open-plan work space. The post holder will be expected to carry out assessments, run depot clinics, manage a caseload and take part in duty shifts. The CMHT works closely with third sector agencies and specialist services. Exceptional communication skills with patients and staff across agencies are essential for this post.

Due to the nature of the role, the post holder would need to be flexible in their approach, be recovery focused and ensure that they embody the UHB's core values and behaviours.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
